Responsibilities
- Provide compassionate, resident-centered care while assisting with activities of daily living, including bathing, dressing, grooming, toileting, feeding, oral hygiene, and mobility.
- Assist residents with repositioning, transfers, and ambulation while promoting comfort, dignity, and independence.
- Obtain and accurately document vital signs, weights, intake and output, and other assigned clinical observations.
- Collect routine specimens as directed.
- Perform restorative nursing activities in accordance with resident care plans.
- Observe residents for changes in physical or emotional condition and promptly report concerns to the licensed nursing staff.
- Respond promptly to call lights and resident requests.
- Assist with admissions, transfers, and discharges.
- Maintain clean, organized, and safe resident rooms and common areas.
- Complete rounding assignments, including safety and comfort checks.
- Accurately document care provided in the electronic medical record.
- Communicate effectively with residents, families, visitors, and members of the interdisciplinary healthcare team.
- Follow all infection prevention, safety, and resident rights policies.
Requirements
- Current Pennsylvania Certified Nursing Assistant (CNA) certification required, or eligibility to work in Pennsylvania with an Ohio or West Virginia credential.
- Current CPR certification preferred.
- Previous long-term care, skilled nursing, rehabilitation, or hospital experience is preferred but not required.
- Compassionate, dependable, and committed to providing exceptional resident care.
- Strong communication, teamwork, and time management skills.
- Ability to work effectively in a fast-paced healthcare environment.
