We deliver support through various programs including community housing, day programs, outpatient treatment, recovery services, education, and employment training. Responsibilities
- Provide support and assist individuals with all activities of daily living, including grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, transferring, ambulation, range of motion, and communicating.
- Check vital signs, check weights, apply creams/ointments, and ensure the comfort, privacy, and dignity of individuals.
- Observe residents for changes in condition or behavior and report these promptly to the Program Director.
- Ensure compliance with DDS, CARF, and all applicable regulatory standards.
- Identify and secure appropriate services for individuals' medical, clinical, and financial needs.
- Transport individuals to and from medical, recreational, and other activities outside the program.
- Maintain daily and monthly progress notes according to regulatory standards.
- Write annual assessments on individual goals as outlined in the ISP and participate in relevant meetings.
- Aid individuals in accessing community resources and recreational opportunities.
- Administer medications according to MAP regulations.
- Maintain core training certifications including CPR/FA, OSHA, CPI, PABC (if applicable), and MAP.
- Perform other duties as assigned.
Requirements
- High school graduate with at least 1 year of experience working with people with developmental or psychiatric disabilities.
- Successful completion of a Massachusetts-approved Nursing Assistant Certification course or equivalent education.
- Current Nursing Assistant Certification in Massachusetts.
- Valid driver's license and one year of driving experience.
- Excellent written and oral communication skills.
- Excellent organizational and problem-solving skills.
- Normal range of eyesight, hearing, and physical abilities to perform job duties.
Qualifications
- Bachelor's degree may substitute for a CNA based on experience.

Pay
The hourly rate range for this position is $20 - $28 per hour.
