Responsibilities
- Provide direct personal care assistance, including bathing, dressing, grooming, toileting, feeding, and ambulation.
- Monitor residents' physical condition and report changes in health status to the appropriate clinical staff.
- Absorb medication observation and medical monitoring as directed by care plans and organizational policies.
- Support residents in developing and maintaining daily living skills and self-help activities.
- Foster positive relationships and healthy interactions among residents and their housemates.
- Ensure the safety and well-being of residents at all times and maintain a safe living environment.
- Assist residents with recreational, social, and community-based activities.
- Accompany residents to scheduled appointments, community outings, and other approved activities.
- Document care provided, resident progress, and significant observations accurately and timely.
- Communicate effectively with residents, families, healthcare providers, and team members.
- Follow individualized care plans and support strategies.
- Maintain compliance with all agency policies, procedures, and regulatory requirements.
- Participate in team meetings, training programs, and ongoing professional development activities.
- Perform other duties as assigned to support resident care and program operations.
Qualifications
- Minimum age requirements is 18 years.
- Valid driver's license, with satisfactory driving record, as defined by agency policy.
- Certification, if required by state licensure as a CNA or Home Health Care Aide in the state in which the aide is to provide care.
- Will be required to obtain DSP Certificate within 6 months of employment (company paid).
- Will be required to obtain CRMA which company will provide training.
- Completion of a recognized home care aide training program or 6 months of recent experience as an aide.
- Must be able to utilize proper body mechanics while lifting and may be expected to lift up 75 lbs.; must be able to sit, stand, and walk for periods of time and bend, stoop, push, pull, and reach.
- Ability to communicate effectively both verbally and in writing.
- Demonstrated compassion, patience, and commitment to person-centered care.
