Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements, and our policies and procedures.

This position is 100% home care providing services in patients’ private homes.

Responsibilities

  • Follows care plan developed by RN.
  • Reports client changes in condition to RN, as described on care plan.
  • Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
  • Assists clients with household tasks including planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
  • Measures and records client’s oral, rectal, and axillary temperatures, pulse respiration, and blood pressure as ordered.
  • Performs range of motion and other simple procedures as an extension of therapy services as assigned.
  • Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record. The Home Health / Hospice Aide shall not administer medication of any kind.

Requirements

  • High School diploma or GED required.
  • Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program (CNA license for CT required).
  • Documentation of one year’s employment as an aide in an institution, home care, or hospice setting.
  • Meets agency hiring criteria as defined in agency minimum standards (e.g., provision of professional references).
  • Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency’s client and their family.
  • Must accept responsibility for maintaining skills, learning, and adhering to agencies policies and procedures on an on-going basis.
  • Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics, and adhere to the Compliance Program.
  • Must have current Driver’s license and reliable transportation to and from work site.

Schedule

Monday - Friday, 8 am - 5 pm

Pay

Position qualifies for a $5,000 sign-on bonus paid out over two years.
