Position Details
This full-time position is 100% home care, providing services in patients’ private homes within the Enfield region. The schedule is Monday through Friday, 8 am to 5 pm. This position qualifies for a $5,000 sign-on bonus paid out over two years.
Responsibilities
- Follows care plan developed by RN.
- Reports client changes in condition to RN, as described on care plan.
- Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
- Assists clients with household tasks including planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
- Measures and records client’s oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure as ordered.
- Performs range of motion and other simple procedures as an extension of therapy services as assigned.
- Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record. The Home Health/Hospice Aide shall not administer medication of any kind.
Requirements
- High School diploma or GED required.
- Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program (CNA license for CT required).
- Documentation of one year’s employment as an aide in an institution, home care, or hospice setting.
- Meets agency hiring criteria as defined in agency minimum standards (e.g., provision of professional references).
- Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s client and their family.
- Must accept responsibility for maintaining skills, learning, and adhering to agency policies and procedures on an ongoing basis.
- Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics, and adhere to the Compliance Program.
- Must have a current Driver’s license and reliable transportation to and from work site.
