Position Summary

Are you a compassionate, dedicated healthcare professional who is passionate about helping others live safe, meaningful, and independent lives? We are seeking a Certified Nursing Assistant (CNA) to join our team and provide exceptional support to individuals in residential and community-based settings.

Essential Responsibilities

  • Administer medications and prescribed treatments in accordance with physician orders, agency policies, and regulatory requirements.

  • Provide direct personal care, including: Bathing, Grooming, Dressing, Toileting, Oral hygiene, Meal assistance.

  • Assist individuals with activities of daily living (ADLs) while promoting independence and dignity.

  • Support mobility needs, including lifting, transferring, repositioning, and ambulation assistance.

  • Implement approved behavioral support strategies and crisis intervention techniques when necessary.

  • Participate in required CPI interventions and other physical support activities as trained.

  • Accompany and support individuals during community activities, appointments, recreational outings, and social events.

  • Provide services in a variety of settings, including residential homes, community locations, hospitals, and healthcare facilities.

  • Assist in assessing individual skill levels and monitoring environmental factors that impact health and safety.

  • Support the implementation of Individual Support Plans (ISPs) and care plans.

  • Maintain required staffing ratios and supervision levels as identified in individual plans.

  • Document progress, services provided, and outcomes related to ISP goals.

  • Participate in team meetings, care planning, and ongoing program development.

Health, Safety, and Environmental Responsibilities

  • Maintain a clean, sanitary, safe, and welcoming environment.

  • Monitor for health, safety, maintenance, or environmental concerns and report issues promptly.

  • Collaborate with the RN and Program Director to ensure quality care coordination and a safe living environment.

  • Promote the rights, dignity, independence, and well-being of all individuals served.

Qualifications

  • Current Certified Nursing Assistant (CNA) certification in good standing with the applicable State Board of Nursing.

  • High School Diploma or GED.

  • Valid driver's license.

  • Reliable transportation.

  • Safe driving record and ability to transport individuals when required.

  • Physical Stamina & Mobility: Meets the physical demands associated with direct support services, lifting, transferring, and active engagement.

  • Time Management & Organization: Balances multiple responsibilities while maintaining quality care and regulatory compliance.

Physical Demands & Working Conditions

  • Regular standing, walking, bending, reaching, twisting, stooping, and climbing stairs.

  • Ability to lift, transfer, and support individuals weighing 50 pounds or more, with or without assistance.

  • Ability to sit for meetings, training sessions, and documentation activities.

  • Ability to operate a motor vehicle and transport individuals safely.

  • Work Environment: Residential homes and community-based settings. Hospitals, medical offices, and healthcare facilities. Office environments with standard office equipment. Multiple work locations as assigned.

  • Equipment and Technology: Computers and electronic documentation systems. Microsoft Office applications. Cellular and multi-line telephones. Copiers, fax machines, scanners, and other office equipment.

Compliance & Professional Expectations

  • Follow all OSHA requirements and workplace safety standards.

  • Comply with HIPAA and maintain strict confidentiality.

  • Adhere to agency policies and procedures, including Fire Safety, Human Resources, Fiscal, and Administrative policies.

  • Maintain knowledge of Bloodborne Pathogens, Right-to-Know regulations, Security practices, and Fire & Safety procedures.

  • Attend all required trainings, certifications, meetings, and professional development activities.

  • Maintain a professional demeanor and exercise sound judgment in all aspects of job performance.

  • Demonstrate commitment to the organization's mission, values, and person-centered approach to care.
