About the role

This position, under the supervision and direction of a licensed nurse, is responsible for preparing, administering, and recording specific oral, nasal, transdermal, rectal, inhalant, and ophthalmic scheduled and PRN medications to the residents of Lakeland Health Care Center (LHCC).

Please note: This position requires an active CNA on the Wisconsin State Nurse Aide Registry, along with completion of the medication aide program for passing medications in a skilled-nursing facility (not CBRF).

Responsibilities

  • Maintain in-depth knowledge of medication actions, effects on body systems, and side effects in relation to geriatric residents.
  • Administer and document scheduled medications prescribed for routine administration according to established LHCC medication administration policies and procedures.
  • Understand and effectively carry out verbal and/or written instructions.
  • Recognize changes in residents’ conditions and communicate those changes to the Unit RN or LPN.
  • Take and record residents’ blood pressure and pulse as scheduled and in conjunction with prescribed medication therapy; report significant changes to the Unit RN or LPN.
  • Report changes in resident's physical and/or mental status to the Unit RN or LPN immediately.
  • Provide personal hygiene, including oral care, bathing, hair shampooing, shaving, skin care, nail care, hand washing, and assist with toileting, bowel and bladder retraining, and perineal care.
  • Assist/perform resident mobility transfers, ambulation, position changes, and range of motion.
  • Serve resident meals and assist residents during meals; interpret modified diet menus and prepare accordingly.
  • Clean and sanitize food preparation utensils, dishes, equipment, and resident serving/dining areas.
  • Monitor refrigerators for proper labeling, dating, storing, rotating, and use of foods; order food supplies from the main kitchen.
  • Employ appropriate assistive equipment such as gait belts, mechanical lifts, and correct body mechanics when lifting, positioning, and/or transferring residents.
  • Document cares given, such as temperature, pulse, respiration, weight, intake/output, and ambulation on flow sheets and required electronic charting.
  • Assist residents in maintaining cleanliness and neatness of rooms, including making beds and disposing of soiled linens and clothing.
  • Review Certified Nursing Assistant (CNA) Summary Cards for assigned residents daily and discuss suggested changes with Nurse Manager; assist with pre-staffing information collection.
  • Help establish/update plans of care (Kardex) for residents by communicating with the Unit Manager and interdisciplinary team.
  • Use standard precautions for infection control and isolation techniques, including the use of a particulate mask.
  • Perform simple treatments as assigned by the Unit RN or LPN within the limits of ability, training, and guidelines of the Wisconsin Administrative Code Chapter HFS129.
  • Participate in at least twelve (12) hours of in-service education programs annually to maintain certification on the Wisconsin State Registry; attend mandatory in-services.
  • Obtain four (4) hours of Certified Medication Assistant (CMA) in-service instruction per year.
  • Deliver care in compliance with the Resident Bill of Rights by maintaining confidentiality and respecting residents’ rights and privacy.
  • Assist with orientation of new CNAs and CNA student clinical experiences.
  • Communicate problems or concerns with team members to ensure efficient handling of issues affecting resident care or facility operations.
  • Respond to emergency situations per LHCC Emergency Response Plan, including resident emergencies such as bleeding, falls, seizures, and choking.
  • Assist with post-mortem care and the admission of new residents; label residents’ personal items.
  • Participate in Quality Improvement activities.
  • Assist residents with medical appointments, including preparation, transportation, and monitoring during appointments.
  • Demonstrate a commitment to county safety and risk management efforts.

Requirements

  • High school diploma or equivalent competencies.
  • Two to three years of relevant prior experience.
  • Knowledge of medical terminology and basic arithmetic skills.
  • Wisconsin Nursing Assistant Certification and active status on the State Registry for Nursing Attendants.
  • State of Wisconsin Medication Administration Certification.
  • Criminal background check free of convictions of abuse of individuals or misappropriation of property in a formal caretaking setting.
  • Ability to perform manual tasks associated with personal care of the elderly.
  • Knowledge and understanding of Resident Bill of Rights.
  • Ability to work independently and as part of a team.
  • Good oral and written communication skills.

Work Environment

  • Regular exposure to moving mechanical parts and fumes or airborne particles.
  • Frequent exposure to toxic or caustic chemicals.
  • Occasional exposure to wet and/or humid conditions, risk of electrical shock, and risk of radiation.
  • Moderate noise level in the work environment.

Physical Demands

  • Constant use of bilateral upper extremities, above/below waist and above/below shoulder height.
  • Constant walking and standing.
  • Constant use of hands and fingers to manipulate objects or tend to resident needs.
  • Constant lifting and moving up to 25 pounds.
  • Frequent lifting and moving up to 75 pounds with aid of provided equipment or a second person.
  • Frequent lifting, pushing, or pulling 50 pounds unassisted with bending and twisting of trunk.
  • Ability to communicate with residents using talking and hearing.
  • Ability to identify resident care needs through hearing and seeing.

"Constant" means at least 2/3's of the shift. "Frequent" means at least 1/3 of the shift.

Benefits

  • Scheduled Time Off (STO): Accrued based on straight-time hours paid each pay period.
    • 10 days per year after 1 year of Walworth County service.
    • 15 days per year after 5 years of Walworth County service.
    • 20 days per year after 15 years of Walworth County service.
    • Certain positions may qualify for additional STO based on continuous related experience.
  • Personal Time Off: 64 hours of personal time off as a lump sum at the beginning of each year (pro-rated for part-time and newly-hired employees). Remaining hours may be moved to the extended sick leave bank or paid out at year-end.
  • Extended Sick Leave Bank: Used for time off due to serious health conditions requiring three or more consecutive days off or during an approved leave of absence.
  • Holidays: Ten paid holidays each year (pro-rated for part-time employees).
  • Insurance:
    • Eligibility for Health, Dental, Life, Vision, Short-Term Disability, and Long-Term Disability insurance benefits on the first of the month following 30 days of service for employees hired into a .75 FTE or higher position.
    • Employees hired into a .50 FTE or higher position are eligible for all benefits except Health insurance.
    • Employees hired into a .49 FTE or less position are eligible for Short-Term Disability only.
    • Health Insurance: Choice of Tier 1 (standard co-pay plan) or Tier 2 (High Deductible Health Plan with a Health Savings Account). The County contributes a lump sum to the HSA for Tier 2.
