About the role
The Clinical Health Advocate (Certified Medical Assistant) is an integral part of our care team, owning patient relationships and driving a collaborative care plan that impacts patients' lives meaningfully. This is a part-time (up to 24 hours/week) hybrid role, combining clinical and administrative responsibilities. You'll spend time in the field, traveling to see patients in Aroostook County, or working from an office in Presque Isle, with remote work options. General work hours are Monday through Friday from 8am-5pm ET.
Here’s what you’ll do:
- Clinical:
- Obtain and document vital signs (blood pressure, temperature, pulse, respirations, weight, height) in the EHR
- Perform venipuncture and capillary blood collection for laboratory testing following proper phlebotomy protocols
- Collect, process, and ready specimens for laboratory analysis
- Administer immunizations and injections (as able) and per physician orders while maintaining accurate vaccine records
- Absorb pertinent environmental advantages/disadvantages impacting patients' well-being and complete physical and social screenings as assigned by the provider
- Educate patients on treatment plans, medications, and follow-up care as directed by providers
- Document patient interactions, clinical data, and care activities accurately in the electronic health record
- Carry out Pop Health care gap closures efforts and sprints as needed per payer contract
- Ensure proper sterilization and maintenance of medical equipment
- Follow infection control protocols and maintain compliance with safety standards
- Be available for field and urgent deployment within your market region, even on unplanned field days
- Connect folks with the various Galileo services and teams, including clinical and administrative functions
- Administrative:
- Represent the organization professionally, embodying our mission, and creating meaningful, lasting relationships with all stakeholders
- Owning overall empaneled patient chart management, prepping charts medically and demographically to ensure readiness and thoroughness of care can be performed smoothly and efficiently
- Run-the-list in partnership with the care team to ensure all orders, care coordination, and constant communication are seamless across each empaneled patient's care plan
- Maintain exam room, mobile unit, and/or office cleanliness, organization, and adequate supply levels
- Manage inventory of clinical supplies and medications, placing orders as needed
- Routinely route inbound mail appropriately, sending patient letters or medical equipment as needed
- Coordinate with insurances to submit and complete prior authorizations and referrals
- Request, follow up, and file medical records with local hospital systems, PCPs, and area HIEs
- Submit Pop Health documents to maximize compliance with payer contracts as needed
- Communicate with patients via text, video, or phone as needed for care coordination, scheduling, and reactive needs
- Participate in projects and events as assigned by the Director of Operations
About You
We would love to hear from you if you have the following or equivalent experience:
- At least 2 years clinical experience as a Licensed Practical Nurse or Certified Medical Assistant
- Experience with phlebotomy is a must
- Experience with injections (vaccines) is preferred
- Reliable access to personal transportation
- Strong people skills and the ability to connect with anyone

Compensation
The hourly rate is $27/hour.
