Certified Home Health Agency Registered Nurse (CHHA RN)

We are currently looking for a Fee for Service Certified Home Health Agency Registered Nurse (CHHA RN) to join our team, offering flexible schedules and hours, patient location choices, and a great work/life balance.

Responsibilities

• Conduct home visits providing skilled clinical assessments, patient education, and direct nursing care to patients in their own homes.
• Coordinate skilled services within the patient’s interdisciplinary team as well as direct the patient’s paraprofessional services.
• Assess patient and family to determine home care needs at the start of care and at regular intervals.
• Initiate Plan-of-Care and make necessary changes as patient's needs evolve.
• Evaluate and provide treatment to patients in their homes.
• Instruct and advise patients and/or families in their care.
• Administer medications and treatments as prescribed by physician.
• Identify and implement discharge planning needs as part of care development prior to discharge.

Requirements

• Valid and current New York State Registered Nurse License and Registration.
• Current Driver’s License.
• Organized and patient-oriented.
• Able to work independently with good oral and written communication skills.
• Minimum of 2 years of clinical nursing experience.
• Certified Home Health Agency (CHHA) experience preferred.
• Competency with Microsoft Office and a reliable mode of transportation to conduct home visits.

Qualifications

The ideal RN candidate is a dedicated, compassionate, and confident clinician with strong organizational, communication, written, and customer service skills. A BSN degree, prior CHHA experience, and a working knowledge of Home Care Home Base EMR are preferable.
