Role Overview: A Certified Diabetes Patient Educator delivers virtual education on diabetes treatments and medication management to patients via phone, video, and email. Responsibilities Include Providing one-on-one patient education through various communication channelsEducating patients on diabetes management strategies and treatment optionsSupporting medication adherence and persistence through direct interactionMaintaining accurate documentation and adhering to program guidelines Qualifications And Skills Bachelor’s degree and/or BSN3+ years of experience as a Certified Diabetes Educator (CDE) or CDCESDeep understanding of diabetes care principles with direct patient experienceProficiency in MS Office Suite (Teams, Word, Excel, PowerPoint, SharePoint)Excellent communication and documentation skillsAbility to work independently in a fast-paced environment
