About the role

Coordinate Referrals.
Support Operations.
Ensure Timely Response.

Responsibilities

  • Manage all aspects of referral acceptance and/or response.
  • Closely coordinate with local Gentiva family brand to ensure smooth operations and exceptional service to referral sources.
  • Work closely with region, area sales and operations leadership, and agency personnel to manage and analyze referral responses, including acceptance, non-admissions, and pending patients.
  • Communicate regularly with referral sources, including doctors’ offices, hospitals, and other entities, to provide updates and resolve any pending issues.
  • Maximize efficiencies with vendors and referral partners to ensure a streamlined intake process.
  • Participate in meetings, committees, and special projects to continuously improve processes and outcomes.
  • Maintain compliance with all company policies, procedures, and applicable federal and state regulations.

Requirements

  • Strong customer service skills with the ability to communicate effectively and professionally.
  • Excellent organizational skills and the ability to multitask in a fast-paced environment.
  • Experience working in an interdisciplinary setting with the ability to maintain business and clinical files in an orderly fashion.
  • High school diploma or equivalent with customer service experience.
  • Able to work collaboratively with internal teams and external partners.
  • Compliance with annual training requirements.
