About the role
The Centralized Intake Coordinator reports to the Manager of Centralized Intake and is responsible for ensuring all referrals are received, processed, and communicated promptly via various platforms. This role manages all aspects of referral acceptance and/or response, coordinating closely with local Gentiva family brand to ensure smooth operations and exceptional service to referral sources.
Responsibilities
- Work closely with region, area sales and operations leadership, and agency personnel to manage and analyze referral responses, including acceptance, non-admissions, and pending patients.
- Serve as the lead coordinator of all referrals and maintain accountability for all aspects associated with acceptance and/or response to eReferrals.
- Communicate regularly with referral sources, including doctors' offices, hospitals, and other entities, to provide updates and resolve any pending issues.
- Maximize efficiencies with vendors and referral partners to ensure a streamlined intake process.
- Participate in meetings, committees, and special projects to continuously improve processes and outcomes.
- Maintain compliance with all company policies, procedures, and applicable federal and state regulations.
Requirements
- Strong customer service skills with the ability to communicate effectively and professionally.
- Excellent organizational skills and the ability to multitask in a fast-paced environment.
- Experience working in an interdisciplinary setting with the ability to maintain business and clinical files in an orderly fashion.
- A high school diploma or equivalent with customer service experience.
- Ability to work collaboratively with internal teams and external partners.
- Compliance with annual training requirements.

Preferred Qualifications
- Bachelor’s degree in business, social work, psychology, nursing, or equivalent business experience.
- Hospice or medical office experience.
- LPN/LVN certification.
