Central Intake Nurse LPN
Job Description
The Central Intake Nurse LPN coordinates and facilitates the delivery of homecare services in conjunction with the homecare team, hospital discharge planning staff and other referral sources. The Central Intake Nurse LPN facilitates referrals, coordinates skilled and non-skilled services, and other community resources through collection and input of data, while demonstrating positive interrelationships and service excellence in the performance of duties by meeting or exceeding the expectations of internal and external service groups.
Education And Credentials
- High School Diploma or GED required.
- NYS Drivers License upon hire required.
- 1 year of experience in Medical/Surgical Nursing required.
- 1 year of experience in a Home Care setting preferred.
- Licensed Practical Nurse upon hire required.
Working Conditions
- Weight Requirement - Medium (up to 50 lbs)
- Standard Hours Bi-Weekly: 80.25
- Weekend/Holiday Requirement: Yes
- On Call Required: No
- Scheduled Work Hours: 8a-4:30p
- Work Arrangement: Onsite
