Full-time position. Monday - Friday, one weekend day a month. Grant funded position. Must have reliable transportation to travel to two local shelters in New London.

Responsibilities

  • Provides clinical and technical support for primary care providers on a daily basis.
  • Implements all planned care items using electronic tools to ensure efficient and accurate workflow.
  • Monitors equipment and performs clinical testing according to OSHA and CLIA standards.
  • Liaises with patients, families, specialists, and all members of the health care team.
  • Ensures continuity and coordination of care in a patient-centered medical home in collaboration with the primary care nurse and provider.
  • Applies evidence-based guidelines from major regulatory and specialty groups; performs pre-visit preparation for all primary care patients.
  • Conducts “e-huddles” utilizing the Planned Care Dashboard to ensure complete and thorough primary care visits.
  • Prepares patients to be seen by provider using established criteria based on the nature of the visit (e.g., sick, physical exam, well-child care).
  • Accurately documents chief complaint and results from evidence-based screening tests.
  • Accurately performs and documents patient vital signs.
  • Independently and accurately performs appropriate CLIA-waived tests, EKG, hearing, and vision tests.
  • Prioritizes work responsibilities to decrease waiting time and collaborates with providers and staff to facilitate patient flow.
  • Schedules tests and referrals for specialty care in accordance with provider orders in a timely manner.
  • Maintains a clean, orderly, and safe working environment; oversees maintenance of supplies and medical logs.
  • Sets up and stocks exam rooms; cleans rooms at the end of each day.
  • Adheres to all infection control standards, follows Universal Precautions, and complies with safety policies.
  • Maintains yearly CPR certification as required for all clinical employees.
  • Exhibits excellent customer service skills to external and internal customers with a focus on quality care and teamwork.
  • Complies with agency-mandated in-service training workshops (e.g., EOC, Working Safely, Abuse).
  • Assists with provider clinic support such as routine medication administration and immunizations under delegation.

Requirements

For LPN Candidates

  • Completion of an LPN program.
  • Current CT LPN License.
  • CPR certification (American Red Cross CPR for the Professional Rescuer and AED or American Heart Association BLS/AED for Healthcare Providers).
  • Skills in EKG use, appointment scheduling, word processing, computer skills, and familiarity with medical testing procedures.
  • Sound organizational skills and accuracy at all levels.
  • Excellent communication and customer service skills.
  • Team-focused with demonstrated leadership qualities.

For Medical Assistant Candidates

  • High school diploma or equivalency.
  • Completion of a Medical Assistant course from an accredited/recognized school.
  • Medical Assistant board certification (AAMA, NHA) or registration (AMT), or achieved within 90 days of hire.
  • CPR certification (American Red Cross CPR for the Professional Rescuer and AED or American Heart Association BLS/AED for Healthcare Providers).
  • Skills in EKG use, appointment scheduling, word processing, computer skills, and familiarity with medical testing procedures.
  • Sound organizational skills and accuracy at all levels.
  • Excellent communication and customer service skills.
  • Team-focused with demonstrated leadership qualities.

Preferred Skills

  • Bilingual.
  • Expert knowledge of the electronic health record.

Physical Requirements / Work Environment

  • Requires manual dexterity to perform clinical tasks and moderate physical activity including standing, walking, and lifting.
  • Potential exposure to bloodborne pathogens and/or bio-hazardous materials.
  • Frequent and alternating standing, walking, and sitting.
  • Occasional bending, squatting, kneeling, and lifting up to 25 pounds.
  • Daily and continuous computer and telephone use.

Additional Qualifications

  • Communicates clinical/client information to providers accurately.
  • Accurately takes messages and communicates on the telephone.
  • Collaborates effectively with medical assistants, providers, and nursing staff.
  • Access to clients’ charts and health care information; maintains client confidentiality according to CHC policies.
