Center Care Team - Registered Nurse

In our medical clinics, we provide comprehensive primary care for senior adults with Medicare Advantage plans, delivering improved quality, better patient experience, and lower total cost of care.
Reporting to the Center Manager, the Registered Nurse (RN) has daily responsibility in direct patient care and education and fosters collaborative relationships with providers and other clinical operations staff. This position is ideal for an experienced nurse who enjoys performing daily patient care, has a passion for working with the senior community, and desires a day shift/Monday-Friday schedule.
Responsibilities
- Adhere to the clinical team workflows.
- Perform daily staff nursing activities, including phone and in-person patient triage and patient education.
- Collaborate with Providers and the Center Manager to identify opportunities to improve clinical performance of back-office personnel and functions.
- Develop, implement, and monitor the effectiveness of performance improvement activities.
- Perform regular monitoring activities to ensure compliance with national and organizational standards of care.
- Clinically prepare the market for internal and external quality audits and help lead the center response to any clinical deficiencies found.
- Maintain current knowledge of state regulations for nursing, medicine, and pharmacy as it relates to scope of practice for back-office personnel.
- Identify and provide appropriate resources to educate patients, their families, and staff members on clinical conditions and processes.
- Oversee clinical quality and patient safety through training and education of the back-office personnel and administrative staff.
- Communicate effectively in all areas of practice, including motivating and engaging colleagues around a common goal.
- Ensure that clinical processes, policies, and procedures are adhered to in the center.
Requirements
- AA/AS degree in Nursing required; BA/BS degree in Nursing (BSN) or Healthcare Administration preferred.
- Valid, active Registered Nurse (RN) license in state(s) of employment required.
- Minimum of 5 years’ current direct nursing/medical work experience required.
- Work in a community-based clinic, public health department, or specialization with the senior population preferred.
- BLS certification for healthcare providers required.
- Problem-solving orientation and eagerness to identify process gaps and implement practical solutions in a matrixed organization.
- Proficient PC skills.
- Fluency in Spanish or other languages spoken by people in the communities we serve is helpful.

- Strong knowledge of healthcare functions, including clinical functions, practice safety processes, reimbursement practices, and quality improvement initiatives.
- Track record of developing and implementing processes for providing excellent customer service.
- Superior clinical/patient care skills.
- Thorough, in-depth knowledge of clinical procedures, legal rules and regulations, and best practices in healthcare.
- Skilled in observation, training, and evaluating clinical/patient care competencies.
