Case Worker

Location: Baylor College of Medicine, Houston, TX

About the role

The Case Worker will provide help in accessing available resources, developing care plans, information referral, and advocacy for clients. The role works with clinicians and other staff to ensure clients have access to a continuum of care addressing health care needs and critical resources such as therapy, psychiatry services, basic needs, entitlement benefits, and long‑term care.

Responsibilities

• Work closely with clinic staff as well as city and county agencies to provide support services to clients with mental health, physical health, and/or substance‑use problems.
• Perform assessments of client needs and, together with the treatment team and client, develop a negotiated care plan.
• Evaluate and revise care plans as indicated, documenting changes in progress notes and communicating updates to necessary team members.
• Build and maintain therapeutic relationships with clients through engagement and negotiated goal setting.
• Serve as client advocate to community service providers to facilitate client access to needed services.
• Maintain client case files, including all required reports and statistics, to ensure coordinated services and document program effectiveness.
• Perform other job‑related duties as assigned.

Minimum Qualifications

• Bachelor’s degree in social work, psychology, human services, or a related field.
• Three years of relevant experience.

Preferred Qualifications

• Master’s degree in social work.
• Licensed Master Social Worker (LMSW).
