PURPOSE OF POSITION
Responsible for overseeing all case work services. Ensures the completion of all required paperwork and full compliance with all City and State RHY regulations and standards. Reports To Program Supervisor
Specific Responsibilities
- Complete Initial Service Plan Review & Case Conferences for clients/families & facilitate additional family meetings and/or program visits.
- Cook Counseling Specialists regarding their specific responsibilities supporting case management services.
- Provide aftercare services to clients for 90-days post discharge.
- Maintain case records & ensure necessary documents are present on all current residents in their 90-day aftercare period.
- Facilitate weekly force field/team meeting.
- Conducts initial intake interviews of prospective residents in the absence of Intake Coordinator.
- Work collaboratively with Program Coordinator, Counseling Specialists and Intake Coordinator to ensure quality of service delivery.
- Ability to work efficiently and independently.
- Highly organized.
- Attend & Participate in QI/QA & program admin meetings.
Qualifications
- Bachelor degree in a related field is required
- MSW preferred

WORKING CONDITIONS
Working conditions vary between office work and field visits.
