Another Chance Drug and Alcohol Rehab Center of Portland offers evidence-based treatment programs to help individuals heal and experience life to the fullest. We create a unique and safe environment to address the specific needs of anyone suffering from drug or alcohol addiction.

Key Responsibilities

• Conduct needs assessments related to housing, employment, medical care, insurance, and legal obligations
• Coordinate with probation officers, DHS, housing providers, and community resources
• Absorb clients with insurance navigation and benefit activation
• Support discharge planning and transition of care
• Maintain accurate documentation in the electronic health record
• Participate in staffing and treatment team collaboration
• Advocate for clients while maintaining appropriate professional boundaries

Qualifications

• Experience in behavioral health, addiction treatment, or human services preferred
• QMHA, CADC, or equivalent experience preferred but not required
• Strong organizational and follow-through skills
• Ability to manage multiple client needs simultaneously
• Clear and professional communication skills
• Preferred knowledge of Washington County and Multnomah County resources
• Experience working with justice-involved populations
• Familiarity with Medicaid and OHP systems
