About the Role
The Case Manager works with residents of the Jewish Board’s Domestic Violence Programs, providing trauma-focused services to help residents heal from past abuse, create a safe environment for themselves and their children, and achieve personalized recovery goals.
Responsibilities
- Carry a caseload of residents and provide services within program regulations and agency mission.
- Complete client assessments, including intake, risk, and overall functioning evaluations.
- Support residents in meeting individual goals through person-centered and collaborative service planning.
- Advocate on behalf of residents with other service providers and link them to community-based treatment.
- Develop safety plans and provide individual and family counseling.
- Make referrals for medical, mental health, and substance abuse services.
- Run groups under supervision and participate in weekly case conferences.
- Participate in intake, assessment, discharge, and follow-up conferences.
- Attend regular staff meetings, trainings, and offsite meetings as required.
- Plan and execute special and holiday events.
- File incident reports and complete timely documentation of all resident interactions, including progress notes, service plans, and assessments.
- Facilitate monthly roommate meetings when appropriate.
Requirements
- BA degree in Social Work (BSW) or a related field.
- Minimum of one year of experience working with Domestic Violence survivors.
- Experience working with populations affected by mental illness and substance use.
- Compassion, patience, and respect for vulnerable individuals.
- Willingness to work with families affected by Domestic Violence.
- Bilingual-Spanish preferred.
Skills
- Excellent resident engagement and verbal/written communication skills.
- Strong attention to detail.
- Ability to work independently and as part of a team.
- Familiarity with Windows environment and willingness to learn new software.
- Ability to travel in the community with residents and engage in physical activities.
- Dexterity to read paper/electronic documents and perform data entry.
