Case Manager - St. Bridget's

About the role

Catholic Charities of the Archdiocese of Newark is one of New Jersey’s largest non-profit social service agencies with programs and facilities serving Bergen, Essex, Hudson, and Union counties. We are currently seeking a Full Time Case Manager for our St. Bridget's Residence Shelter, located in Newark, NJ.

Responsibilities

• Assist clients with appropriate service referrals.
• Develop service plans to enable clients to meet their potential goals.
• Link individual clients to services with program activities suitable for clients' needs.
• Ensure continuity of care and assure clients are maintaining productivity.

Requirements

• Bachelor's Degree required.
• 2-3 years of related experience required.
