Job Duties

• Assisting clients with appropriate service referrals
• Developing a service plan to enable clients to meet their potential goals
• Linking individual clients to services with program activities suitable for clients' needs
• Ensuring continuity of care and assuring clients maintain productivity

Job Requirements

• Bachelor's Degree required
• 2-3 years related experience required
