Case Manager - Social Worker (LMSW/LCSW)

About the Role

The Inpatient Social Worker coordinates safe patient discharges, enhances patient well-being and outcomes, and ensures efficient healthcare service utilization. This role involves collaborating with clinical teams and medical providers to assess patient needs, develop discharge plans—particularly for high-risk patients—and implement these plans effectively. Staying current with healthcare regulations, payer requirements, and discharge planning best practices is essential.

Responsibilities

• Conduct initial and ongoing patient assessments for care coordination and discharge planning.
• Develop and execute tailored discharge plans, especially for high-risk individuals.
• Collaborate closely with healthcare teams to ensure seamless implementation of discharge strategies.
• Maintain knowledge of healthcare trends, regulations, and payer policies related to patient care and discharge processes.

Requirements

• Master's degree in Social Work (LMSW required, LCSW/LCSW-C preferred).
• At least 2 years of hospital social work experience, including post-graduate internship; current employees may have certain requirements waived.
• Valid Maryland Social Work License appropriate to education level.
