Case Manager Social Worker - LMSW

Join a growing healthcare system at LifeBridge Health's Northwest Hospital as an Inpatient Social Worker, collaborating with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention for high-risk inpatients. Responsibilities

• Provide patient and family advocacy.

Requirements

• Seasoned professional knowledge equivalent to a Master's degree, with expertise in more than one discipline.
• Master's in Social Work (required; waived for current LifeBridge Social Workers).
• LMSW required; LCSW/LCSW-C preferred.
• 3-5 years of experience.
• MD Social Work License commensurate with level of education.
• Ability to follow verbal instructions and communicate effectively both verbally and in writing.

Pay

$29.00 to $47.91 per hour, based on experience.
