About the role
Case Manager Social Worker (LMSW) position within the Care Management department at Carroll Hospital in Westminster, MD.
Responsibilities
- Provide comprehensive case management services to patients and their families.
- Coordinate care plans and facilitate communication among healthcare providers, patients, and community resources.
- Conduct psychosocial assessments and develop individualized care plans.
- Assist patients with navigating healthcare systems, insurance, and social services.
- Advocate for patients’ needs and ensure continuity of care.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
Requirements
- Master’s degree in Social Work (MSW) from an accredited program.
- Licensed Master Social Worker (LMSW) in the state of Maryland or eligibility for licensure.
- Experience in case management, healthcare, or social services preferred.
- Strong assessment, communication, and organizational skills.
- Ability to work effectively in a fast-paced healthcare environment.
