About the Role

The Case Manager Social Worker (LBSW) provides leadership in coordinating post-acute care services and delivering psychosocial support to patients and families.

Responsibilities

• Gather comprehensive information to develop individualized care plans.
• Participate in inter-disciplinary collaborations.
• Incorporate social, cultural, emotional, and spiritual factors into assessments and discharge planning.
• Deliver crisis intervention.
• Document assessments and interventions.
• Collaborate with care teams and third-party payors to ensure timely utilization of resources.
• Organize discharge rounds.
• Educate staff on bed utilization and payment policies.
• Contribute to program development and quality improvement projects.

Requirements

• Bachelor’s degree in Social Work.
• LBSW license in Maryland.
• Minimum of 3 years of experience (5+ years preferred).
• Strong communication and organizational skills.
• Adherence to social work ethics.
