About the role

A Case Manager Social Worker (LBSW) coordinates post-acute care and provides psychosocial support to patients and families.

Responsibilities

• Lead healthcare team efforts in patient care coordination.
• Assess patient needs through comprehensive information gathering.
• Develop and implement psychosocial intervention plans.
• Collaborate with inter-disciplinary teams and community resources.
• Provide crisis intervention and counseling.
• Document assessments, interventions, and progress.
• Coordinate discharge planning and ensure smooth transitions.
• Communicate with payors and care teams to optimize resource utilization.
• Educate staff on bed utilization, length of stay, and care levels.
• Participate in staff development, quality improvement projects, and departmental committees.

Requirements

• Bachelor’s degree in Social Work.
• LBSW license in Maryland.
• Minimum of 3 years of experience (5+ years preferred).
• Strong communication, organizational, and ethical practice skills.
• Knowledge of community resources and compliance with social work standards.
