About the role

The HSS Case Manager works independently and collaboratively with a multi-disciplinary team to ensure excellent service provision to older adults 55 and over. The role provides individualized social services, physical and mental health services, referrals to community partners, and general support for healthful daily living.

This position is based at the Mary and Gary Wellness Center (1525 Fourth Ave) and conducts outreach within the Wellness Center to inform and educate older adults on relevant subjects. The Case Manager develops a thorough understanding of the Coordinated Entry System (CES) and Clarity (HMIS) and educates clients on the Client Code of Conduct expectations to promote safety at the Wellness Center.

Our flagship Gary & Mary West Senior Wellness Center in downtown San Diego operates 365 days a year and offers integrated care, including health and social services, housing navigation, enrichment activities, and lifelong learning. Last fiscal year, we provided over 1.5 million meals and coordinated services to over 13,265 older adults.

Responsibilities

  • Connects clients to in-house and community resources, acting as a liaison between service providers and clients.
  • Provides information on community health and welfare resources, including referrals to Serving Seniors Health Educators.
  • Updates and manages community resources listings.
  • Adheres to established policies and procedures, providing excellent customer service in an interdisciplinary setting.
  • Provides advocacy and outreach to the community and stakeholders.
  • Manages a caseload dependent on walk-in and appointment hours.
  • Develops and implements care plans for clients’ unmet needs, including referrals to community-based agencies.
  • Conducts Universal Senior Assessments as applicable.
  • Represents Serving Seniors at community meetings focused on older adults.
  • Communicates effectively with multidisciplinary team members through regular coordination.
  • Completes timely and accurate documentation of client interactions, referrals, and data entry into software programs (Case Worthy, Teams).
  • Complies with budgetary guidelines.
  • Performs other duties as assigned.

Requirements

  • Bachelor’s degree preferred or equivalent combination of education and experience; may require a degree in a specialized field (e.g., social work).
  • Bilingual (Spanish) preferred.

Physical Demands and Work Environment

The ideal candidate must be able to complete all physical requirements of the job with or without reasonable accommodation. The role requires remaining stationary for extended periods while meeting with clients and involves exposure to the normal risks of an office environment.

Pay

$25 - $26 per hour, dependent on experience.
