Case Manager - RN - Per Diem

About the role

This role requires critical thinking, effective communication, and decisive judgment to build positive relationships and lead others. The incumbent will provide education and resources to support care progression, service utilization, appropriate levels of care, and safe patient transitions for patients, families, and healthcare teams.

• Provide leadership in case management practices

Requirements

• Minimum of one year in hospital-based Case Management or five years in healthcare
• Graduate of an accredited nursing program
• Basic Life Support (BLS) Healthcare Provider certification required upon hire
