RN Case Manager - Per Diem

Responsibilities

• Establish safe, individualized discharge plans to achieve quality healthcare outcomes and maximize reimbursement through proficient utilization management services.
• Collaborate proactively with patients, families, and healthcare teams to ensure timely and appropriate care.

Qualifications

• Graduate of an accredited nursing school.
• Minimum of three years clinical experience, with two years in Utilization Review, Utilization Management, or Case Management preferred.
• Knowledge of social and physical factors affecting discharge and community resources.
• Current Registered Nurse license in the state.
• Proficiency in computer applications including word processing and data management.
• Ability to provide age-appropriate care based on patient needs.
