CASE MANAGER RN-HOME HEALTH

Responsibilities

• Assesses admission paperwork and plan of care for completeness, and coordinates corrections as needed.
• Captures care of assigned patients and patient care conferences among a multi-disciplinary team.
• Schedules field staff visits in absence of the scheduling coordinator.
• Demonstrates working knowledge of current Homecare regulations and departmental policies.
• Obtains payor authorization and continually communicates with payor sources and field staff to ensure authorization for services.
• Receives patient referrals from outside sources and prepares demographic and admission information for field personnel.
• Inputs information into EMR (Electronic Medical Record) after determining appropriateness for admission.
• Performs other duties as needed and/or assigned.

Qualifications

• Demonstrated excellent oral and written communication skills.
• Computer proficient.
• Must be detail oriented and organized.
• Ability to establish priorities, work independently and with a team, and communicate effectively with multiple team members.
• Ability to provide professional, courteous, a positive attitude, and to be respectful of others.
• All post high school education must have been obtained from a recognized College or University.
• Associates Nursing Required, Bachelors Nursing Preferred.
• 3 years Home Healthcare or Acute Care experience Required.
