About the role
Enhance patient care through comprehensive case management. Collaborate with healthcare teams to ensure patients receive the best possible outcomes.
Responsibilities
- Develop and implement individualized care plans
- Coordinate with medical staff, social workers, and other healthcare providers
- Monitor patient progress and adjust care plans as necessary
- Provide emotional support to patients and their families
- Document all interactions and care provided in accordance with regulatory standards
Requirements
- Bachelor’s degree in Nursing or related field
- Current RN license in good standing
- At least 2 years of relevant clinical experience
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
Qualifications
- Excellent organizational and time management skills
- Proficient in Microsoft Office Suite
- Experience with electronic health records (EHRs)
Skills
- Critical thinking and problem-solving abilities
- Effective verbal and written communication skills
- Ability to handle sensitive and confidential information
