About the role
Field Based Nurse Case Managers are an important part of the interdisciplinary care team. They are responsible for outreaching and scheduling member face-to-face visits in homes, dialysis clinics, and/or physician offices to support higher risk members needing barrier assessments and face-to-face care plan education. In addition, our field nurses develop trusting relationships with the nephrology practices in their markets and work closely with those practices to support improvement in members' health outcomes.
Responsibilities
- Travel to complete in-home and in-facility visits with members
- Travel to participate in care planning in facilities & provider offices
- Use nursing assessment skills to identify medical, behavioral, social determinants of health gaps or barriers in treatment plans
- Provide condition education, conduct medication reviews, assist with advance planning, and connect to community resources
- Demonstrate ability to establish trusting relationships with members & caregivers and identify innovative strategies to improve and maintain engagement
- Participate in interactions with interdisciplinary team, treating providers, and community resources
- Complete in-patient & post-discharge visits to ensure discharged members receive necessary services and resources to reduce readmissions
- Document and track findings; solid computer and software navigation skills are critical
- Complete and maintain field-based safety trainings
Requirements
- Ability to get licensed in other states as needed
- 3+ years of RN experience, with renal, chronic kidney disease, or dialysis care as a main focus
- Computer proficiency, including strong data entry, using MS Office (Word, Excel, PowerPoint, and Outlook), and typing while navigating a Windows environment
- Reside in a location that can receive a high-speed internet connection or leverage existing high-speed internet service
Qualifications
- BSN or higher level of education (preferred)
- Certified Case Manager (CCM) certification (preferred)
- Diabetic educator experience (preferred)
- Field-based experience going into homes (preferred)
- Telephonic case management experience (preferred)
- Experience working within a managed care environment (preferred)
