RN Case Manager
About the role
The RN Case Manager at Medical City Dallas plays a pivotal role in patient care, coordinating the plan of care, managing the length of stay, and facilitating a smooth transition to post-acute care. They collaborate with the multidisciplinary health care team to ensure quality outcomes and efficient patient throughput.
Responsibilities
- Performs a comprehensive assessment of patients' psychosocial, medical, and discharge needs
- Reassesses the patient's clinical condition and develops strategies to mitigate readmission risks
- Coordinates the plan of care and develops a discharge plan in collaboration with the multidisciplinary team
- Involves patient, family, and significant others in identifying and clarifying needs and expectations
- Evaluates progress using evidence-based tools and approved criteria, and escalates issues through the chain of command
- Makes appropriate referrals to third-party payer and disease management programs
- Facilitates patient throughput with an ongoing focus on effective care transitions
- Demonstrates knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives
- Serves as an advocate for patient's rights, needs, and values
Requirements
- Registered Nurse with current TX state license
- Associate Degree in Nursing or Nursing diploma required, Bachelor's Degree in Nursing preferred
- 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management
- Certification in case management preferred
- InterQual experience preferred
