About the Role

The RN Case Manager provides medically-related case management services to clients in the Housing Services program, helping individuals with behavioral health disorders maintain housing placements. This role involves clinical care responsibilities, including coordination with medical providers, nursing support, and collaboration with a multidisciplinary team to improve overall function and housing placement outcomes.

Responsibilities

  • Provide nurse case management services, including medical care coordination, linkage to health-related resources, monitoring referral outcomes, and developing relationships with the local medical community.
  • Deliver health education and counseling using a strengths-based, client-centered, and trauma-informed approach.
  • Perform clinical tasks such as nursing assessments, treatments, medication administration, and adherence support.
  • Conduct home visits, community visits, and escorts in accordance with clients’ plans of care.
  • Demonstrate positive regard and respect for clients through speech and actions.
  • Involve clients in developing and implementing integrated treatment plans using SMART goals.
  • Complete documentation in the electronic health record, including initial assessments, treatment plans, and progress notes, following established formats and timeframes.
  • Model and reinforce the organization’s core values: dignity, authenticity, hope, justice, passion, and balance.
  • Participate in performance improvement and advocacy activities supporting the organization’s mission.
  • Perform other duties as needed while maintaining compliance with HIPAA and IT security regulations.

Requirements

  • Bachelor’s Degree from an approved School of Nursing.
  • Licensed in Maryland as a Registered Nurse.
  • Personal vehicle and valid Maryland driver’s license.
  • Two years of case management experience preferred.
  • Two years of clinical nursing experience.
  • Experience working with individuals who have behavioral health disorders preferred.

Skills

  • Comfort working with individuals who are homeless and/or low-income.
  • Excellent verbal and written communication skills.
  • Ability to independently deliver clinical nursing services primarily outside the medical clinic.
  • Willingness to integrate principles such as Harm Reduction, Motivational Interviewing, and Housing First into practice.
  • Strong organizational and time management skills.
  • Ability to cope with interruptions and work collaboratively in cross-disciplinary teams.
  • Flexible, positive, and open-minded approach to change.
  • Ability to work with clients who have significant mental health challenges, manage crisis situations, and assess for safety.
  • Experience working with ill, disabled, emotionally upset, and sometimes hostile clients.

This is an essential onsite position primarily based at an agency location.
