RN Case Manager

About the role

The RN Case Manager at Medical City Dallas plays a pivotal role in patient care, leveraging clinical expertise to ensure high-quality, patient-centered care. They collaborate with the multidisciplinary team to manage patient stays, develop discharge plans, and facilitate transitions.

Responsibilities

• Comprehensive assessment of patients' psychosocial, medical, and discharge needs
• Reassess patient conditions and develop strategies to prevent readmissions
• Collaborate with the multidisciplinary team to create and implement discharge plans
• Ensure appropriate post-acute medical needs and levels of care are met
• Make timely referrals to Social Services for identified risks
• Involvement of patient, family, and significant others in goal setting
• Evaluate care progress and escalate issues through the chain of command
• Facilitate patient throughput and ensure efficient care transitions
• Document and communicate care coordination activities
• Act as a liaison between various stakeholders
• Direct activities to meet the needs of under-resourced patients
• Advocate for patient rights and needs

Qualifications

• Registered Nurse with current TX state license
• Associate Degree in Nursing or Nursing diploma required, Bachelor's Degree in Nursing preferred
• 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required
• Certification in case management preferred
• InterQual experience preferred

Schedule

FT Day shift, 8:00am - 4:30pm Monday - Friday with some weekends.
