About the role
The RN Case Manager at Medical City Dallas plays a pivotal role in patient care, leveraging clinical expertise to ensure high-quality, patient-centered care. They collaborate with the multidisciplinary team to manage patient stays, develop discharge plans, and facilitate transitions.
Responsibilities
- Comprehensive assessment of patients' psychosocial, medical, and discharge needs
- Reassess patient conditions and develop strategies to prevent readmissions
- Collaborate with the multidisciplinary team to create and implement discharge plans
- Ensure appropriate post-acute medical needs and levels of care are met
- Make timely referrals to Social Services for identified risks
- Involvement of patient, family, and significant others in goal setting
- Evaluate care progress and escalate issues through the chain of command
- Facilitate patient throughput and ensure efficient care transitions
- Document and communicate care coordination activities
- Act as a liaison between various stakeholders
- Direct activities to meet the needs of under-resourced patients
- Advocate for patient rights and needs
Qualifications
- Registered Nurse with current TX state license
- Associate Degree in Nursing or Nursing diploma required, Bachelor's Degree in Nursing preferred
- 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required
- Certification in case management preferred
- InterQual experience preferred
