Case Manager, RN - Case Management - Per Diem 8 Hour Days WKND (Non-Exempt) (Union)

Responsibilities

• Provides care coordination services, evaluates patient needs, and develops a plan of care.
• Serves as a consultant to the health care team, addressing financial and healthcare issues.
• Participates in interdisciplinary meetings, discharge planning, and patient advocacy.
• Manages a caseload of 18-20 patients, utilizing the online Work list.
• Completes various forms and documentation, including Medicare One Day Stay forms and disposition forms.
• Proposes alternative treatment options and manages resource allocation.
• Ensures compliance with Tenet policies and participates in quality improvement processes.
• Supports patient/family with end-of-life issues and educates them on discharge plans.
• Assists with patient/family education and works with post-acute services.
• Performs InterQual assessments and documents them according to schedule.
• Communicates with payers and completes TARs for processing.
• Notifies supervisors of patients not meeting InterQual criteria.

Requirements

• Associate’s Degree in Nursing
• 3 years of clinical experience
• Knowledge of case management principles and healthcare management
• Problem-solving skills and ability to multitask

Preferred Qualifications

• Bachelor’s Degree in Nursing
• 1 year of case management or utilization review experience within the last three years

Pay

The hourly rate range for this position is $47.31 – $100.00. Offers extend based on experience, qualifications, and other factors.

Schedule

This role is available on a per diem basis, working 8-hour days with weekend availability.
