About the Role

The Case Manager focuses on clinical quality and resource management resulting in defined clinical satisfaction and financial outcomes. They coordinate patient care to a population of different ages within the Singing River Health System. If responsible for Medicaid patients, evaluates Medicaid client needs, builds personalized care plans, and coordinates home and community-based services.

Case Managers must have strong written and oral communication skills, analytical and critical thinking skills, conflict management and team-building skills, and strong organizational skills. The Case Manager must have the ability to perform numerous tasks simultaneously with minimal errors.

Case Managers will participate in the daily discharge planning meetings: addressing barriers that may inhibit discharge; targeting successful transition from one level of care to the next. Case Managers will work to create a seamless line of communication between the inpatient care management team and outpatient case management team.

Responsibilities

  • Coordinate patient care for a diverse age population within Singing River Health System.
  • Evaluate Medicaid client needs, build personalized care plans, and coordinate home and community-based services.
  • Work effectively under pressure, multitask, and be self-directed.
  • Participate in daily discharge planning meetings to address barriers inhibiting discharge and ensure successful transitions between levels of care.
  • Create seamless communication between inpatient and outpatient care management teams.

Requirements

  • Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
  • Currently licensed to practice as a Registered Nurse in the state of Mississippi.
  • Must complete and maintain BCLS certification upon hire and/or transfer.
  • TB certification preferred.
  • Must complete de-escalation training by the end of position orientation (90 days).
  • Two (2) years’ nursing experience in ambulatory or acute care relevant to Case Management.

Schedule

Monday-Friday (8:00am-4:30pm) with holidays and rotating weekends.

Physical Demands

  • Moderately active work involving sitting, moving about the office and facility, and traveling within the SRHS service area.
  • Exerting negligible force frequently to lift, carry, push, pull, or move objects, including the human body.
  • Repetitive motions involving wrists, hands, and fingers (e.g., typing on a computer keyboard).
  • Exposure to body fluids, sputum, and tissues, which may carry infectious disease hazards.
  • Ability to perceive sound at normal speaking levels and make fine discriminations in sound.
  • Close visual acuity and ability to adjust focus between computer monitors and printed materials.
  • Ability to remain active for extended periods without undue fatigue.
  • Flexibility to work assigned schedules, including off days, shifts, or weekends as needed by the hospital.

  • Superior communication skills for receiving and giving information in person and by telephone.
  • Emotional stability to handle high-stress levels.
