About the Role
The Case Manager provides case management services in either the community and/or in office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients.
Responsibilities
- Provide case management services to patients and their families in community or office settings.
- Assist patients and care teams in planning, connecting, outreaching, advocating, and coordinating care.
- Monitor patient needs and progress.
- Help patients access health services, financial assistance, employment, education, social services, and natural supports.
- Assess patient needs and develop individualized treatment plans.
- Document services provided accurately and in a timely manner.
- Promote a safe recovery environment for clients.
Requirements
- High school diploma or GED required.
- Associates Degree or higher in a field related to behavioral health preferred.
- Minimum of six months of recovery from substance use and/or mental health disorders preferred.
- 1-3 years of experience working with the GMH/SU (General Mental Health/Substance Use) population required.
- 1-3 years of experience in a behavioral health position providing outreach and engagement activities required.
- 1-3 years of experience in a medical setting preferred.
- Valid identification required.
- Current AZ Driver’s License (valid and in good standing) preferred.
- 39-month MVR (Motor Vehicle Record) preferred.
- Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.
- Arizona Fingerprint Clearance card.
