Case Manager - Residential Treatment Facility

About the Role

A Recovery Case Manager (RCM) guides clients through their recovery journey by conducting regular one-on-one meetings, developing aftercare plans, and acting as the main point of contact for clients' families, loved ones, and referral sources. The role involves ensuring client accountability, coordinating external needs (e.g., court hearings, benefits applications), and tracking treatment progress through weekly check-ins, step-downs, and discharges.

Responsibilities

• Monitor client compliance with treatment contracts and hold them accountable in the community.
• Serve as the primary contact for clients' families and referral sources, communicating effectively and promptly.
• Attend required meetings, including Morning, RCM, Supervision, and others as needed.
• Engage new clients within 48 hours of admission and introduce them to the recovery process.
• Schedule and conduct family and referral calls to support client progress.
• Assist clients with outside needs, such as court hearings and benefits applications.
• Oversee treatment progress, including executing step-downs and discharges.
• Maintain accurate and timely documentation in the Electronic Medical Records (EMR) system.
• Create realistic aftercare plans and vet visitors and calls for client safety.

Requirements

• High school diploma or GED required.
• Extensive experience in Mental Health and Substance Use Disorder fields.
• Strong time management and multitasking abilities.
• Effective written and oral communication skills.
• Ability to work in a fast-paced environment and make sound decisions.
• Experience coordinating with multiple departments and overseeing client stays.
