Case Manager Registered Nurse - Field (Cumberland/Atlantic County, NJ

About the role
This is a work-from-home position with 85% travel requirements around Cumberland/Atlantic and surrounding counties in New Jersey (within a 50-mile radius of your home). You’ll conduct face-to-face assessments, connect members with vital services, and develop proactive care plans using a holistic, person-centered approach.
Responsibilities
- Conduct 10+ face-to-face assessments per week to evaluate medical needs and overall wellness, considering co-morbidities, multiple diagnoses, and functional limitations.
- Help members access services such as:
- Adult Medical Daycare
- Personal Care Assistance
- Nursing Facility Custodial Care
- Personal Preference Program
- MLTSS Program Enrollment
- Collaborate with your team and supervisor to overcome barriers and achieve goals.
- Use interviewing skills to engage members and uncover their health needs.
- Mentor new team members once you’ve mastered your role.
- Operate at full capacity by month four with comprehensive training and support.
- Develop proactive plans of care to address identified issues and enhance members’ short- and long-term wellness.
- Assess the need for referrals to clinical resources and other interdisciplinary team members.
Requirements
- Must reside in New Jersey; travel will be spread across Cumberland/Atlantic and surrounding counties.
- Must possess reliable transportation and be willing to travel up to 85% of the time (mileage reimbursed).
- Active, unrestricted RN license in New Jersey.
- 1+ years of clinical experience (hospital, home health, or ambulatory care).
- Comfortable working with diverse populations in various settings.
- Strong attention to detail, organizational skills, and ability to work independently.
- Computer proficiency and experience handling medical records.
- Successful completion of the NJ Choice Certification, including:
- Minimum exam score of 80%
- Completion of state training modules
- Field mentoring component
- Ability to work Monday–Friday, 8:00 AM–5:00 PM EST (no weekends or holidays).
Qualifications
- Associate Degree in Nursing required; BSN preferred.
- Assessment, case management, or discharge planning experience (preferred).
- Managed care or crisis intervention background (preferred).
- Strong interviewing and communication skills.
- Proficiency with MS Office Suite and EMR systems.

Pay
The typical pay range for this role is $66,575.00 – $142,576.00 annually, depending on experience, education, geography, and other relevant factors.
Schedule
- Full-time, 40 hours per week
- Monday–Friday, 8:00 AM–5:00 PM EST
- No weekends or holidays
