Case Manager, Project LINK
Position Summary/Responsibilities
The goal at Project LINK is to engage parents in needed services and improve outcomes for newborns and families. Project Link offers trauma-informed treatment, case management, connections to other treatment providers, and specific programming to cut down on barriers to treatment. Project Link case managers will submit for Certified Substance Abuse Counselor (CSAC) credential supervision and co-facilitate substance abuse disorder (SUD) groups. PSH Case Managers in Project LINK are responsible for engaging women in individualized services, treatment group psychoeducation, ASAM assessment and discharge planning services, and obtaining stable housing in the PSH program. They also outreach clients through home visits to engage absent clients, assess safety of parent and child, and intervene when clients are in crisis.
Requirements
- Education: Bachelor's degree in an acceptable human services field, or a bachelor's degree in another field with at least 90 hours of human services field classwork.
- Experience: Working with individuals with a substance use diagnosis is a plus!
- Licenses/Certifications: Eligible for supervision as a Certified Substance Abuse Counselor (CSAC).
- Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
- Safe Driving Record: This position requires transporting clients or driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.
Special Work Conditions
- Sit, stand, walk, bend, stoop, climb, and reach above their head.
- Use of hands for placing and pulling objects.
- Frequent lifting and/or carrying up to 20 pounds and pushing or pulling up to 20 pounds.
- Close vision (computer & paperwork).
- Normal tones of specific hearing.
- Travel up to 10%.
- Transportation of individuals into the community to ensure informed choice of housing options and obtainment of benefits.
Hiring Range
The anticipated base salary for this position is $43,222.07 to $45,890.21. Actual compensation offered within this range will be determined based on the candidate's experience, education, qualifications (knowledge, skills, and abilities), geographic location, complexity of role, and other relevant business considerations. We kindly ask that candidates do not discuss compensation until after an offer of employment is extended.
