Case Manager - PRN
Position
We are seeking a Case Manager to provide fill-in coverage for care coordination services at KSRC Surgery and Inpatient Rehab facility.
Responsibilities
- Assists with psychosocial assessments
- Discharge planning
- Crisis support
- Community resource referrals
- Communication with patients, families, physicians, and hospital staff
Requirements
- Experience in healthcare case management, acute care, post-operative care, in-patient rehab and discharge planning preferred.
- Education: Must have a Master of Social Work or RN license issued by the State of Kansas.

Hours
This flexible PRN position will work on an as-needed basis but must be available to assume full-time responsibilities to provide operational continuity during manager absences including staff support, patient care coordination, and regulatory compliance.
