Position Overview

Responsibilities
- Clinical Interventions/Discharge Planning: Assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.

- Fiscal Management: Promotes cost-effective care within the allotted time frame and ensures that hospital-based services meet patient-related utilization management criteria.

Requirements
- Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.
- Previous experience in Case Management and Discharge Planning preferred.
- CCM Certification preferred.
Qualifications
The ideal candidate will have a strong background in healthcare or a related field, with a focus on clinical interventions and discharge planning. Experience in managing patient care and fiscal aspects of healthcare is highly valued.

Pay
