Case Manager - PRN

Job Summary

The Case Manager at Select Rehabilitation Hospital of Denton is responsible for coordinating health care decisions, ensuring treatment plans improve quality and outcomes, and managing care across the continuum.

Responsibilities

• Clinical Interventions/Discharge Planning: Assess the psychosocial needs of patients and provide intervention as part of the discharge planning process.
• Care Planning Management: Develop and implement comprehensive care plans that meet patient needs and goals.
• Fiscal Management: Manage the financial aspects of patient care, including insurance verification and payment processing.
• Payer/Referral Management: Coordinate with payers and referral sources to ensure timely and appropriate coverage for hospital-based services.

Requirements

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.
• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification preferred.
