Case Manager - PC - Union City

About the role

Catholic Charities of the Archdiocese of Newark (CCAN) is a leading provider of mental health and addiction services in Northern New Jersey. We are currently seeking a Case Manager for our Partial Care department.

Responsibilities

• Provides case management and support to PC clients.
• Facilitates groups.
• Documents according to regulatory standards.
• Carrries a caseload.
• Functions as part of PC Team.
• Interfaces with PC psychiatrist.
• Maintains collateral contacts with family members and providers in the community on behalf of clients.

Requirements

• One of the following:
• Bachelor's degree in a human services field.
