Case Manager - PC - Cranford

About the role

Catholic Charities of the Archdiocese of Newark (CCAN) is seeking a Case Manager for its Partial Care department. The Partial Care program treats severely and persistently mentally ill individuals with potential substance abuse issues. The Case Manager will provide case management and support, facilitate groups, document according to regulatory standards, carry a caseload, function as part of the PC team, interface with the PC psychiatrist, and maintain collateral contacts with family members and community providers.

Responsibilities

• Maintains collateral contacts with family members and providers in the community on behalf of clients.

Requirements

• One of the following:
• Bachelor's degree in a human services field.
