Developmental Pathways (DP) has an opportunity for a Case Manager (CM) - Non-HCBS in our Systems Navigation (SN) Department. This role is responsible for ensuring a timely and responsive intake process for individuals seeking non-waiver program supports. The Non-HCBS Case Manager coordinates eligibility assessments, facilitates access to Non-HCBS programs, connects individuals with appropriate service providers, and supports Intake and Enrollment activities as needed to ensure a smooth and effective service experience.
