Case Manager Navigator, Inside Safe, E6
About the role
The inside Safe Case Manager-Navigator supports the most vulnerable and chronically homeless individuals living in designated encampments through hotel placements and ongoing case management.
Responsibilities
- Engage and provide services to homeless individuals
- Develop trusting relationships to facilitate service delivery
- Collaborate with partners to reduce barriers to permanent housing
- Provide documentation support, case management, and housing navigation
- Ensure clients are linked to necessary benefits
- Identify and place clients into appropriate housing
- Maintain accurate records and follow up with clients
- Attend case conferences, staff meetings, and training sessions
- Work collaboratively with interdisciplinary teams
Requirements
- Bachelor's degree or 2 years equivalent experience in homeless services
- Demonstrated knowledge of and experience with homelessness, mental illness, substance abuse, medical issues, and domestic violence
- Working knowledge of social services agencies in Metro LA (SPA 4) and LA County agencies serving the homeless
- Excellent communication and crisis intervention skills
- Ability to self-motivate and organize time effectively
- Computer literacy and proficiency in a database system
- Passion for working with the homeless population
- Ability to work as part of an interdisciplinary team
Qualifications
