Pay

The pay for this position is $44,850-$58,909 annually. $500.00 sign on bonus

Responsibilities

• Leads service planning and care coordination navigation for children and families receiving mental health services
• Manages referrals
• Provides primary care screening and monitoring
• Serves as a primary liaison with community partners

Requirements

• An Associate’s Degree and two years experience or Bachelor’s Degree
• The ability to establish and maintain supportive relationships with children, youth and families
• Knowledge on how to provide holistic and therapeutic case management services
• Experience working in the mental health field
