Description of Position Be a part of this innovative program that is the first of its kind in Massachusetts. CCBC and the Department of Mental Health (DMH) will work collaboratively in the PACT- Y program to meet the needs of youth and families who want a creative, flexible, dynamic team to support them in achieving their goals. As a Case Manager you will work within a multi-disciplinary team to improve the social, emotional and recreational functioning of youth as they transition to adulthood. The Case Manager will provide opportunities in the community for youth to practice skills, assist with obtaining jobs, explore after-school activities and community resources as well as support goals identified on the team treatment plan.
